You will be asked to provide proof of income in the form of one of the following: 1. Federal tax form 2. One month of paychecks or paystubs 3. W-2 4. Social Security Benefit Statement or other government assistance benefit statement. We will employ a formula to determine your eligibility that will take into account the number of children you sign up and your income. The formula is based on federal poverty level guidelines.
